3 Simple Steps to Enrol Online

For Family Doctors

Video Guide: How to Enrol in CDCC Pilot Scheme as Family Doctor

Step 1: Submit e-Enrolment Form via eHRSS
  • Login to eHRSS platform;
  • Click "Administration";
  • Select "CDCC Pilot Scheme - Doctor Enrolment"
Step 2: Provide the required information via eHRSS platform
  • Agree to the Terms & Conditions of Agreement for Private Doctors, Undertakings and Declaration, and Personal Information Collection Statement;
  • Input required information such as personal particulars, information of clinic and bank information;
  • Indicate the Co-payment fee to be collected from patient per consultation in the Treatment Phase
Step 3: Submit the following supporting documents

Email the following completed forms and supporting documents to us at cdccdoctor@healthbureau.gov.hk

  • Certified true copy of MCHK Annual Practicing Certificate;
  • Duly signed and completed Authority for Payment to a Bank;
  • Certified true copy of bank correspondence (e.g. bank statement) showing the bank name, bank account number and name of the account holder;
  • Certified true copy of Business Registration Certificate (applicable for corporate bank account);
  • Relieving Doctor Enrolment Form (if applicable);
  • Clinic Administrator Enrolment Form (if applicable); and
  • Request Form for Reference Pricing Information of Specified Drugs (if applicable)

Enrolment Checklist

I. ENROLMENT CRITERIA
  1. Holder of a valid Annual Practising Certificate
  2. Enrolled in eHRSS
  3. Listed in Primary Care Directory Enrol in Primary Care Directory
III. INFORMATION REQUIRED FOR E-ENROLMENT
  1. Personal particulars
  2. Professional registration number assigned by the Medical Council of Hong Kong (MCHK)
  3. Business registration number
  4. Bilingual address and telephone number of the clinic(s) intended for providing the services
  5. Bank account number(s) for reimbursement
IV. FORMS AND DOCUMENTS FOR SUBMISSION VIA EMAIL
  1. Certified true copy of MCHK Annual Practising Certificate
  2. Duly signed and completed Authority for Payment to a Bank (available for download in eHRSS)
  3. Certified true copy of bank correspondence (e.g. bank statement within 6 months) showing the bank name, bank account number and name of the account holder
  4. Certified true copy of Business Registration Certificate (applicable for corporate bank account)
  5. Relieving Doctor Enrolment Form (if applicable) (available for download in eHRSS)
  6. Clinic Administrator Enrolment Form (if applicable) (available for download in eHRSS)
  7. Request Form for Reference Pricing Information of Specified Drugs (if applicable) (available for download in eHRSS)

Please note that:-
Separate "Authority for Payment to a Bank" should be used for each bank account.
The application may not be processed by the Government if the registered medical practitioner fail(s) to provide all information and documents as required by the Government.
