Pairing may take place at Family Doctor clinics or DHC/ DHCE.
Eligible individuals may enrol in the Co-care Netork at the Family Doctor clinic which supports participant enrolment (only applicable for cardiovascular disease risk factor screening and management) and directly pair with the Family Doctor visited, provided that the individual Participants have not been previously paired with any Family Doctor.
Participants may also visit their registered DHC/ DHCE to carry out the pairing process or apply for a change of their paired Family Doctor. DHC/ DHCE staff will provide a list of eligible doctors for Participants to select as their Family Doctor, according to their own choice. DHC/ DHCE staff will not recommend or assign any Family Doctor to the Participants. After successful pairing, Participants will be arranged to receive screening and treatment by the selected Family Doctor.
