Eligible individuals without a paired Family Doctor (FD) may contact the FD clinics with the label (Only applicable for Cardiovascular Disease Risk Factor Screening and Management), for directly pairing with the FD visited and enrolling in the CDCC Pilot Scheme. If eligible individuals opt for pairing with FDs without the label, they can visit DHC/ DHCE for FD pairing and enrolment.
