Subsidy & Co-Payment

Screening services	Include screening consultation and related laboratory investigations and examinations The Government will provide partial subsidy to cover the relevant expenses Participant will only need to pay a one-off co-payment fee Cardiovascular Disease Risk Factor Screening : not more than $120 Chronic Hepatitis B Screening : not more than $180 DHC/DHCE will arrange free HBsAg Rapid Diagnostic Test (Applicable to Chronic Hepatitis B Screening only)
Chronic Disease Co-care Scheme*	Consultation and Drug	The Government will provide a partial subsidy for each subsidised consultation, while Participant will only need to pay the co-payment fee determined by Family Doctor. The Government recommended co-payment fee is $150 No additional payment on receiving drugs under the list of Specified Drugs and/or up to 3 days of drugs for episodic illnesses
	Laboratory Investigation	The Government will provide a partial subsidy for each item. Participant only has to pay the co-payment fee
	Dedicated Nurse Clinic and Allied Health Services under District Health Network (Applicable for Cardiovascular Disease Risk Factor Screening and Management only)	The Government will provide a partial subsidy for each subsidised visit Participant only has to pay the co-payment fee (per visit) Nurse Clinic : $80 Optometrist/Physiotherapist : $150 Dietitian/Podiatrist : $380

Note:

Subject to mutual agreement, Family Doctor may charge the Participant for services outside the service scope of the Co-care Network
Under the bi-directional referral mechanism, each attendance of one-off Medicine specialist consultation provided by HA is charged at a standardised fee of $250. In the event that drug prescription is required, a fee of $20 will be charged for each drug item prescribed [4 weeks as chargeable unit (except for self-financed drugs)]. The arrangement also applies to Participants who are HA staff or civil service eligible persons.

Elderly Health Care Voucher (including the reward under Pilot Reward Scheme) is applicable to the cardiovascular disease risk factors and chronic hepatitis B screening and management

Medical fee waiver is not applicable to the cardiovascular disease risk factors and chronic hepatitis B screening and management (including one-off Medicine specialist consultation provided by HA under the bi-directional referral mechanism)

* During the Chronic Disease Co-care Scheme, Participant who fulfils the relevant eligible criteria may receive screening for specified chronic diseases, with the same subsidy and co-payment fee for the screening services
