Doctor-Patient Partnership Incentive Mechanism 
- In order to build a good doctor-patient relationship, encourage patient active engagement in the treatment process, and follow the advice of their family doctor to achieve better health outcomes and status, a collaborative doctor-patient partnership incentive mechanism has been built.
- Incentive payment calculation will start from the Participants' second participant programme year (PPY) onwards.

To be eligible to receive incentive payment, family doctors must fulfil the
pre-requisite requirement by meeting a pre-defined percentage of patients who
satisfactorily controlled their levels of blood sugar and blood pressure. The
calculation of incentive payment is 15% of the total amount derived from the number
of actual subsidised visits in the Chronic Disease Co-care Scheme attended by the patients who have
achieved their incentive targets, the Government subsidy and the Government recommended
co-payment amount.
